    Enter the student’s basic info: first and last name, date of birth, grade, and campus.

    Upload required documents (immunization records, physical forms, or other medical paperwork) as PDFs or images, right from their phone or computer.

    Submit the form and receive an on-screen confirmation that the files were successfully uploaded to the school’s secure inbox.

  1. Open your school’s MyStudentHealthHub link in Safari.
  2. Tap the Share icon Share icon
  3. Scroll down and tap Add to Home Screen. If you don’t see it, tap the three dots labeled “More”.
  4. Optionally rename it (for example, “PRCA Health Forms”).
  5. Tap Add. You’ll now see an icon on your home screen that opens directly to the portal.

This makes MyStudentHealthHub feel like a real app—easy access without installing anything from the App Store.

On Android devices, parents can use Chrome’s Add to Home screen option in the browser menu for a similar experience.
